About Greater Living Home Care

Greater Living Home Care is a Georgia pediatric home nursing agency providing in-home skilled nursing for children under 21 with physical disabilities or medically fragile needs who have Georgia Medicaid. Our team works with families to understand each child’s needs, coordinate care, and develop an individualized Model of Care plan. Program eligibility and services depend on Georgia Medicaid requirements and medical authorization.

Pediatric Skilled Nursing Centered on Your Child

Our care is designed for children whose medical needs require support at home. Depending on the child’s orders and authorized services, care may include support for tracheostomies, ventilators, oxygen, feeding tubes, seizure disorders, suctioning needs, and other complex conditions. Greater Living Home Care’s registered nurses help coordinate care, review the plan, and provide clinical oversight so families have a clear point of contact.

How We Support Georgia Families

First, We Learn Your Child’s Needs

We begin with a conversation about your child’s health needs, daily routines, and family priorities. This initial assessment helps our team understand what questions to address and what information may be needed for the next step.

Next, We Build a Model of Care Plan

Our clinical team works with the family to develop an individualized Model of Care plan. The plan reflects the child’s needs and the services that are ordered and authorized. Families can ask questions and review the plan as care is arranged.

Then, We Coordinate Ongoing Care

Greater Living Home Care provides ongoing coordination and clinical oversight for the care delivered in the home. Nursing documentation and supervisory processes help the care team stay aligned with the child’s plan.

Who May Benefit From Our Services?

Families may contact us when a child under 21 has Georgia Medicaid and a physical disability or medically fragile condition that may require skilled nursing or other authorized support. Examples can include cerebral palsy, spinal conditions, traumatic brain injury, seizure disorders, tracheostomy care, oxygen or ventilator support, and feeding-tube needs. A conversation with our team does not guarantee eligibility. Georgia Medicaid and medical authorization requirements determine which services a child may receive.
